Athletics
We are so excited to begin the 25-26 school year by offering full-length athletic seasons for our middle school students.
Here is information on what needs to be submitted for students to participate in athletics:
Register online through FinalForms:
- Registration must be completed by the parent or legal (court appointed) guardian of the student athlete.
- Parents/guardians can find instructions online: FinalForms Registration website
- Parents/guardians can enter multiple students in one FinalForms session.
- Sports Physical Evaluation Forms may be uploaded directly into FinalForms.
- Students are required to complete a few forms in FinalForms using their district email account as their user name: Edmonds School District FinalForms
Get a sports physical:
A Sport Physical Evaluation Form is required for all athletes prior to their first day of try-outs and/or practices. Forms are valid for two years. In addition, middle school wrestlers must have a healthcare provider complete a weight class permit prior to their first contest each season.

Required Health Forms:
- Students with expired Health Forms from the previous school year will need to submit updated forms to your school nurse BEFORE they will be allowed to try out for a fall sport.
- The following ESD Health Forms are required for students who have a life threatening allergy or need medication administered at school: Allergy Information Form and Authorization for Administration of Medication in School.
Just a reminder about sports fees: The cost is $130 per sport season, plus a one time fee of $15 per year for ASB membership card. Sports fees can be paid starting on September 1st (not before), and fees are waived for students on free/reduced lunch.

Sports practice is every day at 2:45. There is an activity bus that picks up athletes from school at approximately 4:30 pm and takes them home using a predetermined route.
